Title: Outreach and Recovery Health Literacy Support Navigator 

Hours: 40 hours per week - Non Exempt Position

Reports to: Outreach and Recovery Coalition Program Manager


Organization: Street Level Health Project is an Oakland-based non-profit organization dedicated to improving the health and well-being of underserved, urban, immigrant communities in the Bay Area. We prioritize a holistic health framework that provides direct services to empower low-wage workers to access and advocate for resources that strengthen communities.

Health Access Program: The Health Access Program focuses on healthcare and health advocacy, and is a collaborative effort of a broad team of dedicated individuals including a medical director, community health workers, mental health counselors, a nutritionist/herbalist, interpreters, volunteer medical providers, precepting students and external partners. 

Community Outreach Program: In collaboration with Alameda County Health Care Services agency, SLHP has been tasked with providing targeted community outreach and health education to residents of Frutivale and surrounding zip codes, primarily with newly immigrated, low wage workers, day laborers with an emphasis on the Mam community. SLHP will coordinate with ACPHD and coalition members on developing and disseminating public health messages for the target population focused on COVID-19 containment strategies such as masking requirements, contact tracing, and testing. SLHP will also be responsible for disseminating messages through a variety of platforms including, but not limited to, social media, physical collateral, health education events. The coalition is also tasked with providing vaccines and testing via mobile service events. 

Job Responsibilities: 

  • Support the Covid Outreach and Recovery team by: 
  • Creating and/or disseminating existing health literacy materials. Such as infographics, flyers, visuals, video’s, social media, phone banking scripts, etc. 
  • Posting and/or coordinating printing of materials for SLHP and coalition members.
  • Adhering to the necessary budget line items for the program as directed.
  • Track and report back health literacy data necessary for the OMH grant 
  • HAP/Covid Health Navigation: 
  • With the guidance of the HAP manager, initiate and provide referrals to community members from the covid hotline and outreach. 
  • Follow up on referrals and ensure that the community members concerns were addressed 
  • Assist in updating referral resources as needed 
  • SLHP Hotline 
  • Provide systems navigation by connecting community members to services and resources via a hotline. 
  • Ensure recording and tracking of community member information, external and internal referrals in the organizations tracking log. 
  • Return voicemails and messages from the main hotline and covid response hotline, as well as connecting the community to resources. 
  • Assist in updating resources with most recent information for referrals. 


Administration Support:

  • Organize and file patient's chart information
  • Ensure lab reports are printed and filed appropriately (scan lab into Salesforce)
  • Coordinate shredding of medical records and sensitive information
  • Attend and participate in COVID update, coalition and programming meetings, as well as OMH and organizational meetings. 
  • Keep up to date with barriers and community feedback in order to contribute to opportunities for advocacy with systems and partners. 

Organizational Support 

  • Facilitate and participate in team and staff meetings 
  • Help with organizational events as needed (end of year community celebration events, fundraisers, open houses, etc.) 
  • Engage in collective organizational visioning, strategic planning, and programmatic decision-making that contributes to a culture of self-care and work/life balance. 
  • Support development of a growing program with creativity and innovation. 
  • Perform other duties as assigned 

Required Skills and Qualifications:

  • Required fluent bilingual: English and Spanish in written and oral communication, trilingual (Mam) highly encouraged to apply.
  • Experience with the development of content creation for outreach and health education efforts in English and Spanish, as well as editing and proofreading..  
  • Experience with editing and creation platforms such as google suite, microsoft, canva, adobe, etc. As well as social media campaign creation with platforms such as Instagram, Twitter and Facebook. 
  • Comfortable engaging community members in discussion in both English and Spanish.
  • Commitment to serving, empowering, and advocating on behalf of low-income immigrants.
  • High level of independent work, self-direction and initiative, able to work effectively in a hybrid work environment (in office and from home). 
  • Adaptability and creative problem-solving approach.
  • Ability to lift and carry a minimum of 10 lbs and up to 50 lbs with assistance. 


Skills and Experienced Preferred:

  • Knowledge of local, community, and county resources as well as social service and health systems.
  • Strong interest in social justice, social welfare, mental health, food justice, and peer education.
  • Experience working with high-risk, marginalized clients at the intersection of system barriers.
  • Valid CA Drivers License and clean driving record (Record will be verified).

Benefícios

Salary and Benefits: This position is full time 40 hr a week, at a base pay rate of $23/hr. Full Benefits (vision, dental, health) accrual of 10 days of vacation/sick per year, and an addition of 2 weeks paid time off during December closure (after a year of employment).
