Title: Registered Nurse - Triage

Location: 820 Residential Stabilization & Rehabilitation

Schedule: Monday to Friday 9am-5pm (NO WEEKENDS or HOLIDAYS)

Salary Range: $80,000 - $90,000

Overview of Program:

The Program is an OASAS part 820 residential stabilization and rehabilitation program. Consumers receive substance use treatment in accordance with 820 rehabilitation residential services. They also receive services from nurses, psychiatrist and medical providers. The program is designed as a holistic and highly individualized model of treatment and focuses on client-centered and trauma-informed approaches to helping people with substance use and co-occurring mental health disorders.

Overall Responsibilities:

This position is responsible for assessing clients and determining their level of care and need for medical stabilization or rehabilitation by evaluation of substance use, medical, and psychiatric history and its severity, symptoms, and vital signs. Provides direct care to clients in conformance with DEA, DOH and OASAS professional standards, clinician orders, and approved plan of care. Provides nursing care to clients in varying states of substance use and recovery. Performs clinical and administrative health care support services to clients and families according to knowledge, ability, and duties. This person will also work closely with the Transdisciplinary Team, including members from occupational therapy, addiction medicine, psychiatry, casac/casac-t counseling, peer support services, and vocational support. Skilled in handling multiple tasks.


Essential Duties and Responsibilities:

The essential duties include but are not limited to the following activities:

  • Triages referrals & walk-in patients following policy/procedure.
  • Obtains client vital signs, circumstances pertaining to current condition, past medical history, allergies, and current medications that the client is taking and will relay this information to the Provider & Nursing teams.
  • Utilizes nursing assessment skills, incorporates client/family history, diagnostic and physical assessment to appropriately perform intake triage including but not limited to COWS & CIWA.
  • Consistently follows established protocols and clinical guidelines in providing client care including infection control, medication and patient safety guidelines.
  • In collaboration with provider, clinical, practice associate and customer service staff, schedules clients according to triage assessment and provider availability.
  • Responds to all “Triage Nurse Calls, Provider Stat, and Emergencies”.
  • Provides routine monitoring of physical & mental conditions of admitted clients & nursing care with team, assisting as necessary.
  • Assists Nurse Manager and Assistant Director of Nursing with coordination of internal and external work flow (e.g., reviews messages and reports of Providers to ensure that they are addressed in a timely manner, referrals).
  • Understands and can effectively communicate HIPAA compliance, corporate compliance and client confidentiality.
  • Ensures and/or remains in compliance with local, state, and federal regulation, i.e., OASAS, NYS DOH.
  • Maintains appropriate triage logs and tracking information as appropriate to duties.
  • Maintains accurate and timely telephone and written correspondence.
  • Participates in weekly case conferences and transdisciplinary meetings.
  • Provides medication management, administration and teaching when necessary and appropriate
  • Provides ongoing health education and support to clients.
  • Completes other duties as assigned.

Qualifications:

  • New York State licensed Registered Nurse.
  • Fully Covid Vaccinated
  • Current CPR/BLS Certification
  • Working with MICA and/or homeless population and experience in addiction medicine, ambulatory or hospital-based withdrawal service preferred.
  • Must have excellent oral, writing and listening skills.
  • The ability to work independently, prioritize tasks, and create and coordinate schedules.
  • Must have the ability to interface clients as well as all levels of staff.
  • Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
  • Must have experience with electronic health records and knowledge of Microsoft Office, especially Word and Excel.
