Geriatric Care Manager

Corewood Care is a locally owned and operated Care Management and Home Care company. We are one of the few companies in the DMV that offer this integrated service to benefit older adults and their family members. As a team of dedicated professionals, we provide guidance, recommendations, and advocacy to clients and their families navigating the challenges of aging and/or disability. Our Geriatric Care Managers are actively in various community settings and engaging directly with clients, client families and or power of attorneys.

We are looking for an energetic and compassionate Social Worker (LCSW) or Nurse (RN) to join our team. Our Geriatric Care Manager will use assess the client needs to create and coordinate a personalized plan of care for each client. Our Geriatric Care Managers will serve as an advocate/ liaison between client's families and healthcare providers.
