The full-timeFamily Advocate / Patient Navigator / Community Health Worker (CHW) will provide overall family advocacy, guidance and care coordination for neurodiverse individuals and their families seen as part of the Lifespan Program.
- Participate in person in the weekly Lifespan Clinic sessions (Wednesday mornings)
- Follow families (both in person and virtually) throughout the rest of the week to provide advocacy / care coordination services, participate in monthly virtual family support group meetings. There will be some flexibility on the balance of ‘in-person’ and virtual time.
- Advocacy / care coordination services will include, helping patients and families navigate through the complex landscape of health, social and public services and programs, providing advocacy guidance and support including accompanying families during IEP meetings and Impartial Hearings
- Help provide individual and group behavioral and support services
- Help with Lifespan Program coordination and administrative activities, including patient scheduling, verifying insurance eligibility, billing and overall program metric including maintaining rosters of Lifespan patients seen, main issues, and resolution / outcomes - as well as fiscal data collation and reporting.
Salary range / Start date:$50,000 – $55,000 / year / to begin November 2025
Eligibility / Requirements:
- High School diploma / GED
- Authorized to work in U.S.
- Has either 8 or more months experience working as a CHW, or willing to complete approximately 8 - 12 week CHW training program including courses + internship (at no cost to applicant)
Preferred:
- Second language(s)
- Lived experience with autism / neurodiverse individuals
- Familiar with health care services, special education programs, systems of care
