TITLE: Community & Crisis Care Coordinator (Full-Time)

ORGANIZATION:  Philipstown Behavioral Health Hub (“The Hub”)

LOCATION: New York State: Mid-Hudson Valley Region Putnam County (Philipstown / Cold Spring)

SALARY: $55,000

START DATE: August 5, 2024 (Preferred)


Known as “The Hub”, the Philipstown Behavioral Health Hub is a recognized non-profit organization that provides local access to behavioral health and addiction resources in Putnam County, NY (Philipstown region).

Over the past decade, the region has lost many lives to fatal overdoses. Many more residents struggle with issues related to depression, anxiety and addiction. Despite the severity of this crisis, local residents have difficulty finding and accessing critical treatment and prevention resources, many of which are located outside the community.  We are finding that this is particularly true for children in need of in-person appointments.

The Hub was created to give residents a place to turn for local access to these resources. The Hub provides a single point of entry to behavioral health and addiction programs, services and education within the greater Philipstown area.

The Hub is young, just approaching its fifth anniversary.  At this time, however, The Hub is preparing to expand its work in many ways, including more space for programming.  These new opportunities require additional enthusiastic and committed staff members!


  1. We coordinate resources among service providers, to ensure that our residents can get the help they need and no one slips through the cracks.
  2. We host events to educate the community, raise awareness, and reduce stigma associated with issues of mental health and addiction.
  3. We partner with community-based entities, such as local school districts, businesses, law enforcement, medical providers and non-profits to create innovative programming.


The Philipstown Behavioral Health Hub runs on passion.  You have a passion for helping people to help themselves and/or their families through crises, and you are comfortable working with people from a variety of socio-economic backgrounds.  You understand how diversity characterizes and shapes the human experience.  Specifically, you are interested in mental health issues and in the challenges facing individuals with addictions.  

You want to deliver high-quality behavioral health care to the community, and you are proud to participate in the effort to incorporate and sustain best practices regarding clinical and non-clinical care within an organization.

You enjoy work that challenges you on a daily basis and allows you to fully educate and engage both with client-participants and the broader community, including various organizations and institutions.  You are responsible, organized, effective, and efficient.  You are able to perform multiple assignments simultaneously.

You have great communication skills (effective/active listening, verbal and written), and you have the ability to deal tactfully and diplomatically with other Hub employees, Federal, State, and City officials, professional and technical groups, clients, and the general public.


The COMMUNITY & CRISIS CARE COORDINATOR (“CCC”) is a new position at The Hub, reporting to the Executive Director and working with other Care Coordinators. 

  • The CCC is responsible for the management of assistance given to client-participants and matters pertaining to that assistance.  This includes (a) direct support for the work of each care coordinator; (b) being available for consultation with care coordinators in real time; and (c) taking an active collaborative role in cases with some complexity or urgency.  This also includes (a) ensuring positive and effective working relationships with referral entities by knowing their range of services and their expectations regarding the referral process; (b) monitoring and facilitating discharge planning and after-care for participants, as appropriate or necessary; and (c) ensuring the timeliness of The Hub’s follow-through.
  • The CCC is responsible for facilitating effective client-participant data collection, monitoring/reviewing the integrity of this data, and generating reports as needed.  This includes keeping the Executive Director apprised of all current cases in the manner and level of detail requested, as well as ensuring that difficult situations are communicated in real time to the Executive Director.  This also includes participation in case conferences, training, workshops and seminars, as well as meetings with peers as appropriate.
  • The CCC is responsible for initiating, executing and/or sustaining organized activities and programs involving client-participants, community members and community-based organizations and/or institutions.  This includes (a) the generation of ideas for additional services and/or modifications to our existing services and systems; and (b) suggesting best practices regarding consumer satisfaction and client outcomes.
  • The CCC is responsible for researching and generating material to be used in grant proposals relevant to client-participant care and/or Hub programs, activities and events.


  • You possess a Master's degree in social work, psychology, psychiatry, counseling or a related field.
  • You have earned one or more of the following license designations: LCSW, LICSW, LMHC, LMSW, CASAC, or equivalent experience and/or licensing.
  • You have at least two (2) years of relevant experience, including community mental health, addictions/substance abuse, and crisis intervention.  (Previous experience within Putnam, Duchess, Orange, Rockland or Westchester Counties is preferred, but not required.)
  • You are familiar and comfortable with individuals from various cultural backgrounds.
  • You are sensitive to the fundamental components of recovery, in a person- and family-centered approach that incorporates cultural appropriateness.
  • You have a valid driver’s license.
  • You own or have regular access to a vehicle.
  • You are able to work some evening and weekend hours.
  • Residence:  Ideally, you reside in either southern New York State, northern New Jersey, or southern Connecticut.
  • “Lived experience” is preferred but not required.
  • Languages:  English, Spanish (preferred but not required)
