Community HealthCorps at Salud Family Health Centers

Salud Family Health Centers (Salud) is a non-profit, federally qualified health center with a mission to provide quality, comprehensive primary health services to residents of north-central and northeastern Colorado.  We aim to reduce barriers to healthcare for an underserved, often low-income population in an attempt to improve the overall health of the communities we serve. Salud is recruiting 14 full-time Community HealthCorps Members, who are passionate about service and community health, to serve across our clinics. We are currently recruiting for the following clinics: Brighton, Commerce City, Frederick, Fort Collins, Fort Lupton, and Longmont. All Members serve on the Transitions of Care/Home Visit team, acting as liaisons between the clinic, hospital, and community. Members take steps to reduce hospital readmission rates, provide better quality care to Salud patients, reach out to high-risk patients, and provide community based and in-home services. Additionally, Members participate in team meetings, service projects with community partners, team-building activities, professional development, and trainings.
