Missão
Our mission at Lily Home and Hospice is to treat the whole person - mind, body & spirit - by providing compassionate, person-centered care to those in the final stages of an advanced illness. We strive to meet each individual's specific needs throughout their end of life journey. We involve family and caregivers to make the experience more inclusive rather than isolated; and we emphasize quality of life instead of the duration. It is our belief that each person deserves a peaceful, comfortable and dignified transition.

Founded in 2022, Lily Home and Hospice is a family owned and operated health care agency dedicated to providing excellence in end of life care. Our team, comprised of over 50 years of combined hospice experience, includes our medical director, registered hospice nurses, certified nursing assistants, chaplains, social workers, massage therapists, music therapists and volunteers all coming together to carry out the Lily mission in the care of our patients.
Lily Hospice has offices in Eau Claire, Wisconsin and Cedarburg, Wisconsin.
