    Our goal is to provide the highest level of hospice care possible for persons who are terminally ill while offering supportive care for their loved ones. The sooner hospice care is started, the more effectively it can provide the best possible quality of life for you or your loved one’s remaining time. Our hospice care promises: Pain and symptom control are the top priorities of Hospice, assuring that patients have a comfortable, peaceful and natural death. The sooner hospice care is started, the more effective the Hospice team will be in assuring quality of life in the remaining months and weeks. Hospice patients are encouraged to retain control over the decisions affecting their lives. Hospice care at home is based on patient choice, and services are tailored to fit the wishes and needs of each patient and family.

    The Mosaic Life Care Hospice team is dedicated and focused on you or your loved one through the following services:

    • Affirming life by improving the quality of care available for the terminally ill and their families.
    • Providing care in the home or in a nursing facility among familiar surroundings.
    • Providing care for a wide variety of diagnoses including heart disease, dementia, renal failure, chronic obstructive pulmonary disease (COPD), AIDS and cancer.
    • Providing care regardless of ability to pay, thanks to gifts from individuals, organizations, churches, synagogues, businesses, foundations and memorials.

    Offering bereavement follow-up to caregivers of patients we serve.
