Prairie Elder Care is founded on the commitment of providing a life worth living for people living with dementia. Through our small homes and hands on approach, we work to provide the highest quality care in the industry.In addition to our care homes, Prairie Elder Care offers an innovative Day Program where participants enjoy socialization, connections and enjoyable activities, while giving their caregivers respite during the day. Prairie Connect is our latest innovation to further connect the care continuum.

Prairie Connect will provide supportive respite for both the person living with dementia and their families from early to end stages of their journey. The first step for Prairie Connect is for the people ho are not quite ready for day program type services, but still need support and supervision throughout the day. There is a period of time where the person living with dementia is capable of being productive and contributing to the community at large but also a time when their caregivers need a break. So that they can find purpose through utilizing their remaining skills, we are offering the Prairie Connect Work Program. The program will support individuals with job coaches and work partners on site at the Prairie Elder Care Farmstead. This program will be offered at no charge and will provide supervision to set the participants up for success through a job coach and work partners.
