We keep thousands of New Yorker’s healthy - at home - each year. As a Certified Home Health Agency, we deliver comprehensive services to residents of Manhattan, Queens and the Bronx. Additionally, we operate a Long Term Home Health Care Program in Manhattan and the Bronx.

Health & Home Care is on the rise and takes patient referrals from New York City Health and Hospitals Corporation facilities located in Manhattan, Queens and the Bronx. We also welcome referrals that come from the health care community at large, including patients’ families, clinics and local physicians.
