Position Summary:
The Supportive Housing Coordinator – Rapid Re-Housing is responsible for coordinating the implementation, quality improvement, and performance of Rapid Re-Housing (RRH) programs across assigned partner agencies within the Continuum of Care.
Essential Duties and Responsibilities
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily:
- Coordinate Rapid Re-Housing (RRH) program implementation across assigned partner agencies, ensuring consistent intake, service planning, housing stabilization, home visits, recertification, exits, and follow-up services.
- Deliver technical assistance, coaching, and training to RRH program managers and case managers on current program guidance, local written standards, housing-focused practices, and continuous quality improvement.
- Maintain responsive communication with assigned partners and provide follow-up support within established departmental expectations, prioritizing response for urgent requests within 1 business day.
- Track technical assistance trends and recommend strategies to strengthen program implementation and effectiveness.
- Facilitate training, meetings, workgroups, support sessions and case conferencing to provide technical support and enhance practice across the CoC. Develop agendas, materials, and written guidance, facilitating or coordinating a minimum of 1meeting/training/session per month.
- Collaborate with Coordinated Entry, Grants, Data, and Program teams to review program performance, housing outcomes, utilization, and data quality trends. Utilize available information to identify areas of success, opportunities for improvement, support informed decision-making, and develop recommendations that strengthen RRH program effectiveness and participant outcomes.
- Assist with review of case management capacity, rental assistance budgets, and service utilization a minimum of 1 time per month to support effective resource management.
- Conduct monitoring, documentation review, and corrective action follow-up for assigned RRH sub-recipients, and report status monthly.
- Track data and develop strategies to address systems gaps to improve overall program performance.
- Prepare RRH performance reports and data summaries for leadership, funders, and internal teams, delivered on the agreed schedule with zero missed deadlines per reporting period.
- Foster collaborative partnerships with RRH sub-recipient staff to support shared learning, strengthen RRH best practices, and serve as a responsive point of contact for technical assistance, guidance, and problem-solving support.
- Maintain confidentiality and handle participant, agency, and system information in accordance with HSN policies and applicable requirements.
- Perform other duties as assigned that are consistent with the purpose and essential functions of the position.
Qualifications and Requirements
Skills and Abilities
Ability to work with minimal supervision and exercise good judgment; strong verbal and written communication skills; time management, conflict resolution, critical thinking and problem-solving skills; facilitation of adult learning; ability to handle multiple tasks and manage competing demands; and standard office computing skills to include facilitation of virtual meetings on a variety of platforms.
Required Qualifications:
- A bachelor’s degree in Human Services, Social Work, Public Administration, or related field; or equivalent background with a minimum of (3) years of experience working with homelessness, poverty, housing, mental health and human services.
- In lieu of a bachelor’s degree, a minimum of five (5) years of relevant experience in homelessness services, housing programs, case management, or related community-based work may be considered.
- Ability to reliably commute to and from work.
- Ability to understand and apply current program requirements, local written standards, funder expectations, and agency policies.
- Ability to facilitate meetings, trainings, and workgroups in person or virtually.
Preferred Qualifications:
- Training and skilled in Rapid Re-Housing, Motivational Interviewing, housing stabilization, trauma-informed care, and/or fair housing.
- Experience using HMIS and other data systems to support program performance and quality improvement.
- Bilingual (Spanish or Creole) a plus.
Physical Demands
While performing the duties of this job, the employee is regularly required to sit, stand, and occasionally walk or bend, use their hands to type, handle files, and use standard office equipment, and reach for files and materials. The employee must also be able to sit for extended periods at a computer. Additionally, the employee may occasionally be required to lift and/or move objects weighing up to 25 pounds.
Work Environment
This position operates in an office-based environment and community-based settings at partner agency sites, requiring regular local travel. The employee will regularly engage with partner agencies, funders, and provider staff, often addressing sensitive issues associated with homelessness, housing instability, and service delivery compliance. The role demands strong communication and organizational skills, sound judgment, and the ability to manage competing priorities. Work is generally completed between the hours of 8:00 a.m. to 5:00 p.m., generally Monday through Friday. This is a full-time, exempt position requiring 40 hours per week. Additional hours are occasionally required.
Conditions of Employment
- Provision of satisfactory references and completion of background investigation check
- Completion of a pre-employment drug screening as well as post-employment drug or alcohol tests upon reasonable suspicion of use
- Demonstrated computer literacy, which may be assessed through pre-employment testing.
- Completion of HSN new employee orientation and ALL required paperwork prior to reporting to work
- Compliance with all HSN Policies and Procedures.
