    Job description:

    Job Title: Family Peer Advocate

    Location: Brooklyn, NY

    Job Type: Full-Time

    Schedule: Flexible hours between 9:00 AM – 5:00 PM, Monday through Friday

    (Evenings, weekends, and holidays may be required as needed)

    Community Counseling & Mediation (CCM) works to improve the lives of at-risk children and families through innovative and compassionate services. Our expanding network includes licensed outpatient mental health and substance use clinics, award-winning supportive housing programs, HIV and hepatitis prevention services, care coordination, and a wide array of youth development and child-welfare initiatives.

    Using evidence-based and culturally responsive practices, CCM helps families remain together and thrive in their communities.

    Position Overview

    The Family Peer Advocate will use their lived experience to support, empower, and guide parents and caregivers navigating the complex systems associated with their child’s emotional, behavioral, or developmental challenges. This role centers on providing peer-based support, skill-building, advocacy, and connection to services and resources.

    Key Responsibilities

    Support & Engagement

    • Assist families in identifying strengths, challenges, and needs
    • Collaborate with families to develop and monitor Individualized Care Plans (ICP)
    • Facilitate individual peer support sessions and group workshops focused on engagement, empowerment, and system navigation
    • Coach families in shared decision-making and advocacy skills

    System Navigation & Advocacy

    • Provide tools and strategies to help parents/caregivers effectively engage with service systems
    • Accompany families to school meetings, provider appointments, and service planning sessions as needed
    • Encourage active family involvement in treatment planning and community engagement
    • Help families access and use community-based resources

    Skill Building & Empowerment

    • Lead parent education groups focused on skill development and positive parenting practices
    • Promote wellness, resiliency, and peer connection among families
    • Encourage participation in civic, regional, and statewide family leadership initiatives

    Community Outreach & Referral

    • Conduct outreach activities and attend community events to raise program awareness
    • Connect families to needed supports, including referrals for concrete services
    • Help families build sustainable support systems within their own communities

    Documentation & Reporting

    • Maintain accurate and timely documentation of all services and client progress
    • Report outcomes and goals to the program supervisor and Alliance Coordinator

    Qualifications

    • High School Diploma required; Bachelor’s Degree (BA) preferred.
    • Required: Lived experience as a parent or caregiver of a child with social, emotional, developmental, behavioral, or substance use challenges.
    • First-hand experience navigating child-serving systems, such as mental health, special education, child welfare, juvenile justice, or developmental disabilities; or 2–5 years of experience as a family peer advocate.
    • Strong interpersonal skills with the ability to build trust and rapport with families.
    • Basic computer skills, including proficiency with email, Microsoft Word, and Excel.
    • Bilingual in English and Spanish preferred.
