COHME is seeking a motivated, relationship-focused professional to help grow community awareness and referral partnerships throughout the New York metropolitan area. This part-time role is ideal for someone who enjoys networking, building professional relationships, and making a meaningful impact in the lives of older adults and their families.

Key Responsibilities

• • Develop and maintain relationships with community organizations, hospitals, and NORCs.
  • Conduct outreach through in-person visits, phone calls, emails, and educational presentations.
  • Identify and cultivate new referral sources and partnership opportunities.
  • Maintain accurate records of outreach activities and contacts in Conversion Copilot CRM.
  • Collaborate with leadership to leverage new and existing partnerships and share market insights.
  • Attend regular team meetings and provide progress updates.

  • Comfortable networking with healthcare professionals and community partners.
  • Organized, self-motivated, and able to work independently.

Experience in healthcare, senior services, home care, sales, marketing, or community outreach is preferred.
