Position Summary
The Reentry Case Coach provides comprehensive, wrap-around case management and career pathway services to incarcerated individuals and individuals recently released from incarceration. Using an Integrated Case Management approach, the Case Coach supports participants in assessing readiness, identifying strengths and barriers, and developing individualized Career Pathway Reentry Plans that support successful community reintegration.
This role requires regular entry into State Correctional Institutions (SCIs) to recruit, interview, screen, and assess prospective SOAR participants. The position involves approximately 60% travel throughout Pennsylvania, with occasional overnight stays required to support statewide recruitment, assessment, and service delivery.
Key Responsibilities
Case Management & Reentry Support
- Manage a caseload of 35–40 active participants.
- Provide individualized, wrap-around case management services.
- Conduct intake, eligibility screenings, and assessments within correctional facilities.
- Develop and monitor comprehensive reentry case plans addressing employment, housing, education, and behavioral health needs.
- Facilitate regular one-on-one case management sessions.
- Maintain accurate and timely documentation of all client interactions.
- Conduct follow-ups via phone, prison visits, home visits, educational sites, and community locations.
- Advocate for participants’ employment, education, and training opportunities.
Recruitment, Assessment & Engagement
- Conduct in-person recruitment inside State Correctional Institutions.
- Interview, screen, and assess incarcerated individuals to determine program eligibility and readiness.
- Recruit and assess a minimum of 3 candidates per week and support enrollment of 8 participants per month.
- Administer assessments including LS/CMI, BESI, O*NET Career Profiler, and TABE or CASAS.
Coordination & Community Engagement
- Collaborate with jails, prisons, and community-based partners to monitor participant progress.
- Coordinate services with workforce training providers, literacy programs, behavioral health organizations, and benefits coordinators.
- Assist participants with obtaining identification and accessing reentry benefits and services.
- Work with faith-based organizations, government agencies, and workforce development partners.
- Represent the program at agency meetings, community forums, and partner events.
Career Pathway Development
- Develop individualized Career Pathway Plans, including:
- Educational attainment goals
- Vocational training and credentialing
- Career readiness preparation
- Connect participants to job opportunities, resume development, and professional skills training.
Program Oversight & Reporting
- Provide regular updates to the Program Manager.
- Support data collection, reporting, and compliance for grant and funder requirements.
- Assist with program monitoring, evaluation, and continuous improvement efforts.
- Contribute to program development and policy recommendations.
Travel & Schedule Requirements
- Frequent travel to State Correctional Institutions across Pennsylvania.
- Occasional overnight stays required.
- Flexibility with schedule, including early mornings and occasional evenings, to meet facility and program needs.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Counseling, Criminology, or related field preferred.
- Candidates with 8 or more years of relevant experience in reentry services, case management, workforce development, social services, or related fields will be strongly considered in lieu of a degree.
- Master’s degree (MSW) is a plus.
- 2–3 years of case management experience preferred (reentry or justice-involved populations strongly preferred).
- Knowledge of the criminal justice system and reentry resources.
- Strong organizational, documentation, and communication skills.
- Proficiency in Microsoft Office and case management databases.
- Ability to maintain confidentiality and work effectively with justice-involved populations.
- Valid driver’s license and reliable transportation.
- Ability to pass local, state, and federal background checks and meet DOC access requirements.
