Direct Support Professional HS Diploma, Bachelor, Master Degree

Sierra House is seeking an Direct Support Professionals to provide in-home support services directly to youth through evidence-based and data driven methodologies. S/he will also plan, access, develop and/or implement interventions, and assist youth with acquiring, retaining, improving and generalizing the behavioral, self-help, socialization and adaptive skills necessary to function successfully in the home and community. Candidate must be willing to travel to clients’ home. This job requires 90% local travel. Locations to choose from include Monmouth, Ocean, Union, Essex, Passaic, Bergen, Middlesex and Somerset counties.

Responsibilities include but are not limited to:

Writing progress notes/case notes and assessments.

Experience: At least one year of experience in developing and implementing positive behavior and support and ADL/Instrumental skill plans for individuals who have intellectual/developmental disabilities.

Excellent interpersonal and intercultural communication skills;

• Excellent written communication, research, and organizational skills;
• Experience and the ability to develop and complete progress notes;
• Computer proficient;
• Proven ability to think strategically;
• Attention to detail with the capacity to simultaneously manage and work on multiple tasks and projects and set priorities in a challenging workload; and
• Demonstrated capability of building and sustaining relationships with a variety of individuals and business partners.
