Clinical Case Manager

Roots and Wings is a well-respected nonprofit organization based in Morris County, NJ, dedicated to helping youth who have aged out of foster care. Founded in 1999, by Irene DeGrandpre, a CASA volunteer and New Jersey Hero honoree, Roots & Wings began as a community-based volunteer operation that has developed into the only non-profit organization in the region providing holistic services to these youth between the ages of 18 and 24 years of age.

Job Summary

This position offers the opportunity to support the mission of a small-sized nonprofit in a dynamic, team-based environment. The Clinical Case Manager provides comprehensive coaching services to program members, guiding them to achieve their goals utilizing a holistic approach, in order to empower them to become self-sufficient and economically independent upon graduation from the program.

ESSENTIAL JOB RESPONSIBILITIES:

Empowering Aged-Out Foster Youth

• In conjunction with the Senior Program Director, assists in the initial application process, conducts phone screens and interviews with prospective applicants and applies clinical knowledge to determine eligibility
• Responsible for building rapport with new members; creates a trusting environment that encourages social connections
• Conducts various assessments utilizing the EmPath Mobility Mentoring model for economic self-sufficiency to identify new members’ individual goals and objectives
• Assists members with their goal development in all areas leading to self-sufficiency: academic, employment, financial literacy, life skills and mental health needs
• Meets weekly with members to ensure members are progressing with goal attainment and/or problem solve any obstacles that may impede their individual goals
• Utilizes the Mobility Mentoring model to track all members’ progress toward goal objectives

Strengthening Program Curriculum

• Continuously improves Roots & Wings’ RISE program, according to best practices for outcomes attainment with a strong focus on setting goals and measurements, providing program and clinical support to improve the success of our participants
• Assists in the continued development of a Life Skills curriculum and/or facilitates group workshops as needed
• Maintains documentation of all program notes, reports and outcomes needed to comply with agency policies / standards and any grant requirements
• Participates and creates monthly recreational activities with RISE and FLY members to increase social connections amongst members
• Participates in house and roommate meetings monthly to address issues that arise in the shared living environment, utilizing conflict resolution and problem-solving skills
• Visits satellite apartments weekly to ensure all safety and lease regulations are being met and residences are being maintained accordingly.
• Works with Senior Program Director to strengthen FLY programming and curriculum
• Evaluates client progress in the core areas of the program during weekly supervision with the Senior Program Director

Increasing Community Presence / Recruitment

• Supports Senior Program Director in recruitment efforts of new members
• In partnership with Senior Program Director, attends aging out events to increase awareness of RISE program and services provided
• Collaborates with community partners to cultivate local resources for program members. Makes and maintains community connections to provide members with referrals to additional wrap around services

Professional Development Responsibilities / Other:

• Participate in professional development and training to expand knowledge and maintain best practices for serving aging out population
• Performs other duties as assigned

Qualifications

• Master's degree counseling or social work with an LAC/LCPC or LCSW license in the state of NJ
• Completes all necessary CEUs to maintain licensure
• 2+ years of full-time experience in social work or a related field
• Knowledge of foster care / aging-out population and ability to develop rapport with this population
• Ability to work effectively with diverse individuals from various racial, cultural and socio-economic backgrounds
• Knowledge and sensitivity to culturally diverse issues
• Experience with developing and organizing community outreach and linkages
• Valid New Jersey driver’s license
• Ability to work effectively and as a team player, multitask and work autonomously as needed
• Able to prioritize job responsibilities if any crisis occurs
• Strong clinical assessment, intervention skills and relationship building skills as needed
• Able to be flexible with work schedule
• Excellent organizational, communication skills and computer literacy

Job Type: Full-time, in-person

Pay: Competitive based on experience
