Good Shepherd Center is contracted by the Department of Health Services (OHS) to operate a Housing for Health (HFH) program that provides Intensive Case Management Services (ICMS) to vulnerable unhoused individuals and families. The HFH case manager is responsible for providing direct services to vulnerable unhoused individuals and families throughout Los Angeles County, as part of the ICMS program. There is occasional travel for this position. Reliable transportation is required.
Under the direction of the Senior Program Manager, the Case Manager is responsible for providing individual case management to unhoused individuals and families utilizing a theoretical framework incorporating Housing First (HF), Trauma Informed Care (TIC), Motivational Interviewing (Ml), Critical Time Intervention (CTI) and Stages of Change (SoC). This framework provides tools that reflect best practices to motivate the Participants to achieve their housing goals in a non- judgmental manner. The Case Manager will perform specialized tasks including comprehensive case management services such as intake, assessment and goal setting for all Participants. The Case Manager will document all Participant interactions and input that data into the Homeless Management Information System (HMIS) and CHAMP databases.
