    ESSENTIAL FUNCTIONS:

    Under the general supervision of the Parent Engagement Coordinator, the Home Based Home Visitor is responsible for planning and implementation of home-based learning experiences for children and families and promoting of children’s progress across the standards described in the Head Start Early Outcomes Framework: Ages Birth to five, including for children with disabilities and dual-language learners, as appropriate.

    DUTIES AND RESPONSIBILITIES:

    1. Establish and maintain trusting and caring relationships with each child and family in the program.
    2. Conduct Educational, health, and nutritional activities for children and parents
    3. Provide 90 minutes weekly visits for children and their parents.
    4. Plan and facilitate bi-weekly socialization for children and parents.
    5. Partnership with families to develop goals, needs, and interests.
    6. Participate in on-going recruitment of children and complete enrollment for eligible families.
    7. Maintain a caseload of 12 families and assist them in identifying strengths and needs.
    8. Follow and Implement the Curriculum Parents as Teachers.
    9. Complete family partnerships agreements, children's developmental screenings and conduct a quarterly assessment of children's development.
    10. Serve as liaison and act as an advocate among families and community medical and social services.
    11. Maintain accurate records in the Salesforce system and other data spreadsheets that include: health information, Education activities, assessments, evaluations, and other required forms.
    12. Maintain records of child observations and assessments in the Teaching Strategies online System.
    13. Provide case management to families when necessary.
    14. Consult with Education Coordinator and Director of Special Education the results of developmental screenings and concerns with individual children.
    15. Meet the needs of all children include those at risk and children with special needs.
    16. Participate in children’s Evaluations with CPS and Early Intervention.
    17. Meet with the Education Coordinator and the Director of Special Education including other program staff for staffing on individual children.
    18. Make referrals with the support of the Director of Special Education when intervention is necessary.
    19. Participate in the transition process for children and families transitioning into other programs.
    20. Assist with transportation required by parents and children to attend socialization activities as needed.
    21. Attend and participate in staff meetings, parent meetings, in-service meetings, and other meetings as necessary.
    22. Perform record keeping and other functions as assigned and report regularly to the Parent Engagement Coordinator.
    23. Maintain knowledge current in the field, including attendance at seminars, workshops, and 20 hours of professional development each program year.
    24. Complies with Onward Neighborhood House Head Start policies and procedures.
    25. Participates in annual program self-assessment and community assessment.
    26. Attend regular supervision sessions with the supervisor.
    27. Perform other related duties as assigned.

    This position requires the employee to:

      • Regularly walk, talk and hear to ensure children’s safety
      • Regularly lift up to and carry 50 lbs. (average weight of a child)
      • Regularly sit on the floor, hold a child, bend, stoop, kneel, crouch or crawl
      • Regularly transport self to different sites/homes for implementation of program activities, meetings and other agency business
      • Have manual dexterity (to button a child’s coat, teach a child to write his/her name, put puzzles together, etc.)
      • Regularly feel objects, tools or controls and reach with hands and arms
      • Have full vision capabilities—including close vision, distance vision and the ability to adjust focus

      • The noise level in the home/classroom is usually moderate unless children are engaged in an activity that requires a louder volume
      • The noise level on the playground will be high
      • The noise level in the offices will be low

    QUALIFICATIONS:

      • At least a BA in Social Work Required
      • Must have Gateways Family Specialist Credential Level 5 or must obtain by 2026.
      • Bilingual (Spanish/English) required.
      • Valid driver’s license and own transportation required.
