To help further the mission of Harbor, we are looking for a talented Housing & Emergency Financial Assistance Case Manager to join our Community Programs team at the Daniel Driscoll Neponset Health Center.

    Role:

    The Housing and Emergency Financial Assistance Case Manager (HEFACM), Ryan White Programs, provides ongoing support to eligible clients living with HIV/AIDS in the Boston Eligible Metropolitan Areas. This position works as a part of the Client Services team which focuses on working with clients to adhere to medical care and achieve viral suppression.

    In this capacity, the HEFACM works closely with the Social Worker and Program Manager, Housing and Emergency Financial Assistance to help clients sustain affordable, long term housing and obtain short-term emergency financial assistance. The HEFACM also provides housing search and advocacy services to Harbor’s Medical Case Management clients and assists with developing and implementing larger housing search and advocacy service efforts within the community. This is a grant funded position.

    Responsibilities:

    • Assists the Social Work and Program Manager, Housing and Emergency Financial Assistance with reviewing and processing rental assistance and emergency financial assistance applications.
    • Works with housing and emergency financial assistance clients to identify and address barriers disrupting stable housing and emergency needs and coordinates care with and refers clients to appropriate community agencies and medical case management to address these barriers.
    • Collaborates with medical and other staff to develop and implement patient care plans via multidisciplinary case conferences and individual provider meetings.
    • Provides Housing Search and Advocacy services to Harbor’s Medical Case Management clients living with HIV/AIDS.
    • Accompanies Harbor’s Medical Case Management clients to housing related appointments, including interviews, court appeal hearings, apartment viewings, lease signings and other public welfare and community resources for advocacy and support.
    • Assists with developing and implementing expanded Housing Search and Advocacy services to the greater Boston community.
    • Performs program outreach activities and presents housing and emergency financial assistance program information with the Social Worker and Program Manager, Housing and Emergency Financial Assistance to generate referrals and establish and maintain relationships with AIDS Serving Organizations (ASOs).
    • Assists with preparation and co-facilitation of Housing and Emergency Financial Search groups and participates in consumer engagement activities.
    • Contributes to coalition building activities with other Ryan White-funded housing and emergency financial assistance organizations.
    • Attends monthly department meetings, mandatory and non-mandatory trainings, and conferences.
    • Assists in regularly reviewing, revising and updating program forms with Social Worker and Program Manager to best serve clients.
    • Assists the Social Work and Program Manager with gathering data and compiling reports for the Ryan White Program.
    • Assists in the collection and maintenance of demographics and service utilizations and other pertinent data for grant and contract requirements.
    • Assists with managing the Housing and Emergency Financial Assistance budgets, monthly outcomes reporting, policy and procedure updates.
    • Practices under DPH, HRSA HAB, and BPHC case management standards of care.
    • Timely and accurately records necessary and pertinent documentation in client’s medical record.
    • Implements quality improvement strategies.
    • Participates in annual Part A BPHC Site Visits, and HRSA Site visits once every 3-5 years and contributes to preparations of audit. Assumes program coordination duties for both the state and federal Ryan White Programs and any other initiatives developed under the Community Programs Department related to enhancing services and resources to people living with HIV/AIDS.

    Requirements:

    • Bachelor’s degree in Social Work, Human Services or related field required, LSW preferred.
    • Minimum of one year of Case Management experience, experience related to housing or emergency financial assistance preferred
    • May substitute License in Social Work (LSW) , LADC or MSW for one year of experience, or five years of experience in Case Management for the BA/BS
    • BLS (Basic Life Support for Healthcare Providers) Certification and current MA LSW or LADC license if appropriate are required
    • Ability to work independently and cooperatively as part of a multidisciplinary team
    • Ability to prioritize, organize and schedule busy workdays
    • Strong working knowledge of Boston and surrounding community, specifically regarding services for people who are unstably housed or low-income
    • Experience working with people living with HIV/AIDS preferred
    • Bilingual/bicultural in Spanish and/or Haitian Creole preferred
    • Basic business writing skills required, basic skills in effectively using Microsoft Office 365 applications including , Word, Excel, Outlook and Teams required
    • Must be able to travel between Harbor sites in a timely manner; required to accompany clients to other sites such as social service agencies and public welfare agencies
