he HUB Case Manager, who will report to the Community Health Supervisor, will provide face-to-face case management services to clients assigned through the Community HUB network. The HUB brings together community-based, behavioral health, and healthcare organizations to provide culturally responsive care coordination support for community members. The HUB Case Manager will provide direct service referrals and case management to clients with identified Health Related Social Needs and be responsible for community-based care coordination and outreach. In addition, the HUB Case Managerwill be required to maintain client data, accurate documentation of client interactions, community outreach, client/program referrals and engagement with community members.
