    Anthos|Home is a groundbreaking startup dedicated to housing vulnerable New Yorkers. We're seeking a passionate and experienced Referral Coordination Specialist to join our team and make a significant impact on the lives of those in need of stable housing.

    The Role

    The Referral Coordination Specialist connects NYC Health + Hospitals (H+H) referred patients to Anthos|Home’s housing navigation services. The Specialist ensures a smooth intake process, gathers essential documentation, and coordinates closely with shelter and outreach providers to streamline housing transitions. This will require one-on-one interaction with patients and may require meeting with them in the community or in shelter. The Specialist ensures all materials are collected to support timely and complete housing navigation, while also monitoring referral pipeline performance to strengthen program impact.

    Key Responsibilities

    • Referral Engagement: Serve as the first point of contact for all H+H housing referrals. Promptly engage referred participants upon consent to begin documentation collection and establish initial rapport.
    • Work closely with Anthos|Home referral lead to track reporting to meet established department metric and be lead on H & H reporting metrics.
    • Documentation Support: Meet participants in shelters or other community-based settings to gather identity verification, income documents, and subsidy paperwork. Assist clients with recovering or replacing key documents as needed.
    • Shelter and Outreach Coordination: Act as a liaison between Anthos|Home, shelter staff, and outreach workers to avoid duplicative efforts and align on voucher processes including CityFHEPS, Section 8, and other housing subsidies.
    • Packet Readiness Assurance: Assemble a complete, accurate housing packet—including all required documents—for handoff to the broader Housing Navigation team.
    • Targeted Housing Navigation Support: Offer intermittent support on a case-by-case basis to bridge gaps in navigation, such as help with unit applications, appointments, or landlord communication.
    • Referral Quality Monitoring: Track challenges and documentation issues that affect referral processing. Share insights regularly with Anthos|Home leadership and H+H to inform system-level improvements.
    • Ongoing Participant Coordination: Maintain communication with participants throughout their housing journey to ensure continuity and coordination between referral, navigation, and housing placement phases.
    • Internal Collaboration: Work in close partnership with Anthos|Home’s Housing Acquisition, Tenant Coordination & Aftercare, Operations, and Data & Evaluation teams to facilitate smooth transitions from referral to placement.
    • Cross-Agency Coordination: Join biweekly calls with H+H and other stakeholders to review participant progress, flag issues, and promote alignment across systems

    Qualifications and Skills

    The Referral Coordination Specialist must be a people person who loves working with others and solving problems. Bachelor’s degree, and/or 3-5 yrs experience working with people in the housing and human service sector.

    Additional Skills include:

    • Excellent interpersonal skills
    • Some familiarity with the NYC housing market and voucher system
    • Excellent verbal, written, communication and negotiation skills
    • Willingness to travel and make multiple visits in the community
    • Ability to use public transportation or meet the same travel requirements with reasonable accommodation
    • Ability to ascend/descend stairs, as some work locations may not have accessibility to an elevator, or meet the same requirements with reasonable accommodation
    • Experience with community-based outreach
    • Ability to utilize critical thinking skills in decision-making situations, good organizational and record-keeping skills, and good independent judgment
    • Proficiency in MS Word, Salesforce, Excel, and PowerPoint
    • Passion for the mission of the organization
    • Ability to work in a diverse environment

    Compensation and Benefits

    • Competitive Starting Salary: $65,000 - $70,000/year (dependent on experience)
    • Hybrid Work Environment: 2-5 days in office/field per week
    • Tuesday – Saturday schedule a possibility;
