Springwell is seeking an enthusiastic and creative Care Coordinator to join our Housing Department.
The compensation for this role is $30.77/hr for a 37.5 hour work-week.
POSITION DESCRIPTION:
Our Housing program brings care coordination into public and private housing sites for older adults and people with disabilities. This Care Coordinator provides 20 regularly scheduled office hours per week at the assigned site in Watertown. They offer resources, consultation and assistance to all residents of the building. They also plan and run programming for all residents to improve individuals’’ quality of life and foster community.
The Care Coordinator also conducts comprehensive in-home assessments with older adults, helps determine needs and eligibility for government subsidized services, creates care plans and coordinates services. The Care Coordinator communicates and collaborates with housing management staff and community professionals.
Our ideal candidate will have a strong desire to work with older adults and individuals with disabilities, excellent organizational skills and be able manage constant change with a positive attitude.
