    Descrição

    The Adult Protective Services (APS) program provides in-home assessment, advocacy, and case management services to abused, neglected, and exploited older and disabled adults in Essex County. Our mission is to help individuals achieve their goals by providing comprehensive information so they can make informed decisions, along with advocacy and support to ensure they remain in charge of their own lives. Candidates must be energetic and flexible, with experience and a strong interest in engaging older adults.

    Duties and Responsibilities:

    The Case Manager in Adult Protective Services plays a critical role in protecting vulnerable adults and ensuring their safety, dignity, and well-being. Key responsibilities include:

    • Investigating reports of abuse, neglect, and exploitation: Conduct timely investigations and determine the need for appropriate interventions in accordance with established standards of promptness.
    • Providing personal support services and crisis intervention: Offer direct support and crisis intervention to clients, serving as a liaison between clients and service providers.
    • Evaluating and resolving complaints: Assess, investigate, and resolve complaints while ensuring compliance with agency policies and legal requirements.
    • Maintaining documentation and reports: Keep accurate and complete case documentation and submit required reports within designated timeframes.
    • Advocating and coordinating services: Advocate on behalf of clients and coordinate services, including arranging alternative living placements when necessary.
    • Participating in case planning: Collaborate with multidisciplinary professionals to develop and implement individualized case plans.
    • Documenting and summarizing case activity: Maintain thorough and accurate records of assessments, case activities, and service plans.
    • Managing caseload: Maintain an average caseload of approximately 35–45 clients, ensuring timely and effective service delivery.
    • Community outreach and education: Participate in community outreach activities to promote awareness and educate the public about the Adult Protective Services program.
    • Professional development: Maintain professional growth by attending trainings, seminars, and community development programs to strengthen professional knowledge, skills, and competencies.
    • These responsibilities reflect the Case Manager’s vital role in safeguarding vulnerable adults. The position requires a strong understanding of relevant laws and regulations, excellent communication skills, and the ability to work collaboratively within a multidisciplinary team.

    • Communication skills: Demonstrated ability to communicate effectively with clients, families, service providers, and other stakeholders.
    • Crisis intervention: Ability to provide timely support and intervention to clients experiencing crises or emergency situations.
    • Documentation: Maintain accurate, thorough, and up-to-date records of client cases, assessments, and services provided in accordance with agency standards.
    • Collaboration: Work effectively within a multidisciplinary team to ensure coordinated and comprehensive care for clients.
    • Problem-solving: Assess complex situations, identify risks, and determine appropriate interventions to safeguard clients’ safety and well-being.

    Qualifications:

    • Minimum of a Bachelor’s Degree from an accredited institution.
    • Experience working with the elderly and/or disabled adult population, including providing crisis intervention and/or case management services.
    • Successful completion of the Adult Protective Services (APS) Core Training conducted by Rutgers School of Social Work within one year of hire.
    • Computer literacy required, including proficiency with Microsoft Office applications.
    • Possession of a valid New Jersey driver’s license and access to a reliable vehicle is required.
