Community Health Center Southern Oregon

Community Health Center (CHC), with 4 modern clinic facilities located in Ashland, Medford, White City, and Butte Falls, Oregon, seeks qualified successor to current CEO retiring in December, 2013 after 26 years. The organization has an annual budget of over $8 million and approximately 100 employees including physicians, PAs, FNPs, RNs, LCSWs and related support and outreach staff, some with bilingual ability. In 2012, 10,282 unduplicated patients were served during 38,393 visits to the four clinics and three school based health centers. CHC offers core services on a sliding fee scale including primary, preventive, perinatal, and behavioral health care, along with complimentary services such as prescription medication assistance, diagnostic laboratory services, and specialty services. CHC is supported by a network of partnerships including volunteer specialty physicians who provide consultations on site or in nearby private offices. Three full-service area hospitals provide additional in-kind laboratory and diagnostic services, and in-patient care. CHC has been awarded Tier 3 (highest) recognition by the Oregon Health Authority as a Patient-Centered Medical Home (PCMH) for all its clinic sites and School-Based Health Centers (SBHCs). The EPIC electronic medical record system is fully operational and utilized by all providers. Data driven Clinical Quality Improvement procedures are in place.
