Community Health Lab (CHL), an Adloris Foundation program, is a nonprofit organization leveraging technology and community engagement to create social and physical environments that promote good health for all. We believe in the power of co-production, which is a shift away from the traditional model of service user passively receiving from a service provider to an approach that values the experience and contributions of all stakeholders. To aid us in those efforts, CHL is creating frameworks, toolkits, and platforms to help community members and community care teams investigate social determinants of health (SDOH) and develop solutions to improve outcomes.
