L.A. Net, A Project of Community Partners

LA Net is a non-profit organization focused improving access and quality of primary health care services in low-income communities in order to improve the health and well-being of low-income families in Southern California and to reduce health disparities in the region. It pursues this mission through providing resources to community health centers and other community based organizations to support practice improvement, practice-based research, and spread of best practices in primary care and community health. LA Net places practice facilitators in safety net practices to support quality improvement, develops social and behavioral health interventions in collaboration with community health centers and families in the community to address areas of need, and facilitates participatory research in practice and community settings focused on identifying causes and solutions to health disparities in the region. LA Net is a learning network of community health centers and other CBOs that have come together to help build healthier more empowered communities and families. Recent LA Net projects include a study of the management of Obstructive Sleep Apnea in low-income patients, creation and evaluation of a promotor-delivered home visit parenting intervention for immigrant families from Latin America, and development of a quality improvement coaching intervention to support innovations in primary care.
