Care Connection for Aging Services

We make a difference in the lives of people by providing them with opportunities to create positive aging experiences. We operate 21 locations with the vision that the people we serve will live with maximum independence, respect and dignity.

We offer a wide variety of services: nutrition, care management, family caregiver support, in-home services, health education, fitness, recreation, disease management, falls prevention, long-term care ombudsman program, benefits counseling, and Medicare information and counseling, as well as enrollment assistance in the Health Insurance Marketplace.
