Sagepoint Senior Living Services is a non-profit organization, located in La Plata, Maryland that has been providing excellent care and trusted advice to seniors and their families in Southern Maryland for 40 years.

Formerly the CCNRC Family of Care, Sagepoint remains dedicated to providing high quality care and creating an environment that enhances health and wellness for our patients, residents, and participants. Our entire team is committed to maintaining a standard of excellence in rehabilitation and senior services.
