Points of Life

Founded in 1985 as Norwalk Rehabilitation Services/Health and Home Care Resources, the company officially changed its name to Constellation Health Services in 2009 to reflect increased range, both in terms of services offered and geographical reach.

While the scope of services provided by Constellation Health Services is wide, and includes Home Care, Hospice, Private Duty, Geriatric Care Management, and School Based Therapies, our team approach ensures that each and every person, and organization we work with benefits from our combined knowledge and clinical experience.

Constellation Health Services is an affiliate of National Healthcare Associates. Founded in 1984, National HealthCare Associates (NHCA) has grown into a network of 40 skilled nursing and rehabilitation centers and assisted living centers located throughout Connecticut, Maine, Massachusetts, New Hampshire, New Jersey, New York and Vermont.

Hospice Volunteers are vital to the heart and mission of our organization.  They support our patients and their families in the community, meeting them whereever they are.  In addition, they provide ongoing support in the volunteer office as well as bereavement support.
