Community Health Collaborative

Community Health Collaborative (CHC) works to improve quality and access to health care in Philadelphia. Our organization aims to decrease health disparities in the Philadelphia region. By working one-on-one and in small groups with those who want to imrpove their health and quality of life, CHC is able to provide the appropriate services needed to each individual. In addition to providing health-related services, CHC bridges the gaps in social service needs in order to adequately address all areas of life in an integrated manner. CHC offers programs for those with chronic diseases, as well as preventative services and access to healthy food and nutrition education. Furthermore, we offer those in the community the opportunity to grow their own food and interact with their fellow Philadelphians.
