At Heritage Healthcare, care is provided around our belief in preserving the individual’s dignity and ensuring peace and comfort at the end of life. Support to the patient and family is an integral part of the whole hospice experience. The appropriate goals of care involve doing everything possible to achieve the most satisfactory level of physical, emotional, and spiritual comfort possible for all persons facing a life-limiting illness. We offer:

Manages the patient’s pain and symptoms. Symptom management is an important part of hospice care.

Medication regimens are kept as simple as possible with a minimum number of drugs for effective comfort care.

Provides needed drug, medical supplies, and equipment.

Coaches the family on how to care for the patient.

Provides continuing education regarding disease progression and what to expect as the patient nears death.

Assist the patient with emotional, psychosocial, spiritual aspects of dying.

Spiritual support for the patient and family. Chaplain services for those patients and families who wish spiritual counseling before making an end of life decision.

Makes short-term inpatient care available when pain or symptom becomes too difficult to manage at home, or the caregiver needs respite time.

Provides Bereavement Services and counseling to surviving family and friends, as well as to the community as requested.
