Descrição
As the only faith-based, non-profit health system in the area, St. Joseph’s/Candler Health System operates two hospitals with inpatient, outpatient, skilled-acute care, and emergency department facilities. The volunteer will be placed in the emergency department’s medical home program which specifically cares for vulnerable persons who are under or uninsured. A volunteer will serve as a community resource navigator by identifying, educating, and assisting patients in establishing and maintaining a primary care office as well as accessing resources for social determinants of health. The volunteer will also have an opportunity to train and support program interns.
