Help Support Patient Care as a Referral Care Coordinator Volunteer!
Join our team as a Volunteer Referral Care Coordinator and help support the Referral Department and LVIM providers by assisting with chart readiness, documentation tracking, and referral-related follow-up needs.
At LVIM, we are dedicated to empowering the medically underserved of Polk County to live healthier lives by providing access to FREE, high-quality, comprehensive healthcare through a community of caring volunteers. In this role, you will help ensure that providers have the necessary clinical information available before patient appointments, helping reduce delays and support continuity of care.
This opportunity is ideal for someone with a clinical background, such as a retired nurse, medical assistant, or other healthcare professional, who is detail-oriented, comfortable reviewing medical information, and interested in supporting patient care behind the scenes. Responsibilities may include reviewing patient charts, confirming that lab results, imaging reports, diagnostic tests, consult notes, and other supporting records are available, and helping identify missing or pending documentation needed for upcoming appointments.

Responsibilities and Duties
- Review patient charts before specialty care or primary care appointments to help ensure required documentation is complete and available.
- Verify that lab results, imaging reports, diagnostic tests, consult notes, and other supporting records are uploaded into the patient’s chart.
- Identify and track missing or pending documentation needed for upcoming appointments.
- Check provider folders, fax queues, and other communication areas for referral-related documents and prescription or medication-related requests.
- Notify designated Referral Department staff of missing documentation or testing needs and route prescription or medication-related requests to the appropriate provider for review.
Qualifications
- Clinical background preferred, such as a retired nurse, medical assistant, or other healthcare professional.
- Familiarity with medical terminology and clinical documentation, including lab results, imaging reports, diagnostic tests, and consult notes.
- Strong attention to detail and the ability to identify missing, incomplete, or pending documentation.
- Comfortable using computers, electronic health records, fax queues, and other electronic communication systems.
- Ability to maintain patient confidentiality, follow HIPAA guidelines, and communicate professionally with LVIM staff and providers.
