Provide support to hospice patients and staff by conducting phone calls (on Thursday and/or Fridays) to patients/families to assure that they have the necessary supplies, medications, and support that they may need. Information gathered is then relayed to the patient care manager (PCM) or designee. Tuck-in calls are to be made in the agency office at 223 S Herlong. Each tuck-in call is documented by the volunteer. Training will be provided!
