Position Overview: The Homeless Program Outreach & Care Coordinator will provide frontline support to individuals experiencing homelessness by coordinating essential services, conducting outreach, connecting clients to care, and supporting their journey toward wellness, stability, and self-sufficiency. The ideal candidate is passionate about community service and able to dedicate 10–15 hours per week.
This role is ideal for retired social workers, counselors, nurses, community advocates, and individuals with experience in social services or case management who are passionate about serving vulnerable populations.
Key Responsibilities:
- Direct Outreach & Engagement
- Conduct in-person outreach to individuals experiencing homelessness in shelters, encampments, and high-need community areas.
- Build rapport and trust with clients through consistent, compassionate engagement.
- Conduct needs assessments to identify priority support areas.
- Case Management & Care Coordination
- Assist clients in accessing mental health care, medical care, addiction support, transportation, and emergency services.
- Help individuals obtain IDs, benefits, housing assistance, and essential documentation.
- Maintain detailed case notes and update client progress logs.
- Resource Navigation & Program Support
- Connect clients with HTC programs, community partners, and external service providers.
- Coordinate referrals to shelters, food programs, healthcare providers, and crisis services.
- Support distribution of essential items such as hygiene kits, clothing, food, and resource materials.
- Collaboration & Community Partnership
- Work with local nonprofits, shelters, hospitals, outreach teams, and faith-based groups to align services.
- Participate in community meetings and homeless coalition activities.
- Maintain communication with partner agencies to ensure continuity of care.
- Safety Monitoring & Crisis Response
- Identify individuals in crisis and assist in connecting them to emergency support.
- Follow HTC safety protocols and emergency communication procedures.
- Document incidents and escalate critical cases to supervisors as needed.
Qualifications & Skills:
- Bachelor’s degree preferred (Social Work, Psychology, Public Health, Human Services). Master’s degree a plus.
- Experience in case management, counseling, behavioral health, or social services strongly preferred.
- Strong interpersonal and communication skills.
- Ability to engage compassionately with vulnerable populations.
- Organized, reliable, and comfortable documenting client interactions.
- Knowledge of local homeless services and community resources is a plus.
- Ability to maintain confidentiality and adhere to ethical service standards.
