Position Summary

Navigating

Aging Needs LLC (NAN) is seeking a “NAN Navigator” to add to our team. NAN Navigators help support family caregivers and their loved ones who are suffering from Alzheimer’s disease and other afflictions of aging.

Our Navigators are the primary contact point for family caregivers enrolled in the NAN subscription service, ensuring that they have the information, resources, and support they need to be successful caregivers to their loved ones. Our Navigators will be responsible for assessing the caregiver’s and loved one’s needs across a range of health and lifestyles areas, creating and helping maintain a customized care plan to support themselves and their loved one, and developing and nurturing a trusting relationship with each family caregiver.

The NAN Navigator role requires experience and compassion in caring for others, knowledge of Alzheimer’s disease and other diseases of aging, strong organizational skills, timeliness, and attention to detail.

The NAN Navigator will work directly with family caregivers (and indirectly their loved ones), as well as interacting with multiple stakeholders including NAN staff and outside consultants and experts.

The NAN Navigator will report to the Manger of NAN Navigators.

Primary Functions/Responsibilities

The NAN Navigator will work with the NAN Navigator Manager, NAN staff, and outside consultants, to coordinate the ongoing care of family caregivers and their loved ones. Duties include:

FAMILY CAREGIVER SUPPORT:

Schedule and maintain an ongoing series of interactions with family caregivers via video conferencing, telephone, and email, as determined by the terms agreed to with the client providing or caregiver purchasing subscriptions and each caregiver’s needs, preferences, and abilities.

  • Identify the family caregiver’s and loved one’s specific needs via NAN’s comprehensive proprietary assessment tools.
  • Based on information gathered from the assessment, create and maintain a customized “NAN Care Plan” for each family caregiver and loved one.
  • Provide and encourage family caregiver participation in educational opportunities and beneficial training programs.
  • Recommend appropriate experts for the needs of each family caregiver and loved one and coordinate the interactions with and service delivery from these experts.
  • Reassess each family caregiver’s progress and needs through the term of the subscription and update the NAN Care Plan as necessary.
  • Serve as support for technical, communication and coordination issues with the NAN service for the family caregivers as they arise.

ADMINISTRATION SUPPORT:

  • Create and maintain internal documentation for each family caregiver and loved one on an ongoing basis.
  • Create and manage user accounts, calendar scheduling, record-keeping, data-gathering and archiving for family caregivers and their loved ones on an ongoing basis.
  • Help identify and maintain an appropriate referral network of experts and services that are beneficial to the family caregivers and their loved ones, and serve as a liaison to these providers and help coordinate their services.
  • Help create and recommend educational materials, training resources, and other beneficial content and tools as necessary.

Qualifications

  • Master’s degree in social work, sociology, psychology, gerontology, or a related social services field and relevant experience obtained through a practicum, internship, or clinical rotation on an equivalent basis for up to one (1) year of the experience requirements.
  • Bachelor’s degree in social work, sociology, psychology, gerontology or a related social services field and a minimum of two (2) years of relevant experience,
  • Knowledge of Alzheimer’s disease and related disorders and the impact of this illness on the loved one, their family members and especially the family caregiver.
  • Ability to conduct caregiver support in English and Spanish with excellent oral and written communication skills.
  • Knowledge of the range of eldercare related services, goods, private and government programs in the area of service.
  • Experience in out-patient and/or acute care medical settings preferred.
  • Ability to manage multiple priorities; process-oriented with documentation and project management skills.
  • Ability to read, analyze, and interpret advance care documents including living wills, durable power of attorney. “do not resuscitate” orders, and other related documents.
  • Strong interpersonal relationship skills.
  • Proficiency in Windows and Microsoft Office applications.
  • Basic computer skills including data entry (database), MS Word, Excel.
  • Valid Driver’s License; reliable car with current insurance.
  • Ability to work independently, while collaborating with other team members.
  • The primary worksite- will be home office, equipped and maintained to NAN brand standards. There will be a need to meet in-person in the Orlando area office up to two times a year. In addition, other short-term overnight, in-state travel may be required several times per year.
