Descripción

The Platform of Hope is seeking applicants immediately for the position of a full-time, salaried Program Manager. The Program Manager will be an important enhancement to our 6- person team, preferably starting in June 2024. This person will work closely with our team of POH Program Managers to ensure care coordination for families and support administration, communications and programming as needed. The Platform of Hope utilizes a hybrid model and expects the Program Manager to be available for in-person office days at least one day a week, as well as some additional hours on one Saturday a month and select weeknights. Please note it is required that this person be bilingual in English and Spanish.


Responsibilities:

1.    Families: Establish and maintain supportive, professional relationships with participant families (65%)

  • Establish supportive, professional relationships with all participant families
  • Provide one-on-one monthly coaching to portfolio of families (estimated 20-25 families) to support the identification and pursuit of holistic goals across housing, education, finances, education, healthcare, etc., partnering with Program Managers and Program Administrator on care as needed
  • Support families with referrals to services/ resources, including identifying emergency services as needed as well as cultivating family-driven, long-term solutions
  • As part of the interdisciplinary team of care, participate in care conference as needed, respecting confidentiality while supporting participant movement towards goals
  • Ensure /maintain all confidentiality agreements and “need to know” requirements (adhering to child abuse and neglect reporting and other requirements in support of the safety of families and the communities)
  • Ensure appropriate documentation and coordination of one-on-one monthly meetings
  • Work with POH team to administer assessments to families and track program outputs
  • Document program elements such as curricula, individual stories and group outcomes


2. Programming: Collaborate in planning and implementing program activities, including: collaborations with partners; evening workshops; Saturday Family sessions  (15% of time)

  • Attend weekly staff meetings to coordinate communications with families, workshops, referrals/ resources and ensure internal coordination 
  • Support the implementation of the programming agenda in partnership with POH team and partners 
  • Support the hosting of events, as needed, primarily the Family Saturdays (3rd Saturday of the month, mix of remote and in-person)
  • Support in identifying/ cultivating families for leadership opportunities on Advisory Board and Planning Committee, as well as support coordination of their participation, as needed


3.  External: Lead the development of new partnerships for referrals and additional services to families; represent POH in advocacy work centered on families (10% of time)

  • Identify affinity systems change campaigns 
  • Seek and recruit services from other partners for necessary services such as legal, employment as well as additional housing/early childcare/youth development/ health 
  • Participate in forums and coalitions to help raise POH profile and find opportunities to share family voices


4. Internal operations: active participation in record-keeping and data collection, in partnership with POH staff (10%)

  • Support in evaluation plan for all families, including collection of all assessments in partnership with Program Coordinator (radar charts, socio-emotional assessments, journal entries)
  • Ensure record-keeping on family issues in all relevant databases, as well as appropriate documentation of case notes and confidential materials;
  • Assist team in tracking internal collection of benchmarks for the program (attendance at events, attendance for one-on-one meetings, referrals to outside partners, etc).


Reporting Relationship and Schedule 

The Program Manager will report to the Platform of Hope Executive Director but work closely with the three Program Managers and Program Administrator. Please note that some evening and weekend work is required as part of this position in order to best accommodate families’ schedules. It is suggested that this person would work one day per week in the office and remotely for the remainder, subject to public health guidance– however, we are willing to be flexible on schedule and hours so that the position works well for the employee! Please note that, as per the policy of our fiscal sponsor Social Good Fund, this person must be fully vaccinated from COVID-19.

Candidate Qualifications

The Platform of Hope is seeking a team member with the following required characteristics:

  • Experience within the nonprofit sector and working with families of color-- particularly Black and brown communities, including first generation immigrants
  • A “family first” approach to providing services that centers communities in decision making and values families’ innate knowledge and abilities, upholding families’ dignity and self-determination as core pillars
  • An emphasis on trust and authentic relationship-building with families, with compassionate and non-judgmental approach
  • Ability and willingness to commit to assessment plan and documentation as well as respect for confidentiality
  • Openness and approachability, with ability to connect to people of diverse backgrounds
  • Deep analysis of systems of oppression and interest in anti-racism work
  • Ability to work remotely, with ability or/ commitment to learn utilizing zoom calls and google drive (POH will provide laptop and software)
  • Experience and comfort in meeting and tracking goals
  • Bilingual in English and Spanish required

  • Committed to base building among communities to decide on goals and advocate together with the vision of larger systems change
  • Someone who already has a network in place/is aware of the network for social services and has lived or professional experience in accessing

About the Platform of Hope

The Platform of Hope is a pilot program that began in September 2018 to combat the displacement of low-income families of color from Adams Morgan/ Ward 1 by supporting families in accessing resources and connecting to their community. Our program works to change the paradigm of traditional social services delivery to center the voices of families in making holistic decisions about their life, as well as uphold social capital between families as a key factor for long term success. POH’s model looks at the integration of housing, education, employment, family stability, physical health, mental health, etc. to partner with families to identify their goals and then self-direct plans to accomplish those goals. POH was formed and continues to have active participation from our founding partners, four anchor institutions in Ward 1: Jubilee Housing, Jubilee JumpStart, Mary’s Center/ Briya Public Charter School and Sitar Arts Center. We work with founding partners collaboratively to assist families in pursuing goals (particularly through referrals), as well as develop additional programming based on families’ needs. We intentionally facilitate the building of strong social networks among participating families to highlight families’ innate resourcefulness and knowledge. POH believes in systems change that centers the lived experience of families and we hope to further our advocacy work as the program continues.
