Social Services Coordinator

The Town of Harwich Council on Aging is looking to hire a qualified Social Services Coordinator. This position provides professional and direct social services to the community's senior citizens.

Essential functions of the job include

• Assess and facilitate services and programs for Elders in Harwich who may be unserved or under-served and monitor any changes in the individual's situation
• Coordinate with the Town's public safety agencies, Elder Services agency
• Assist with fuel assistance and food stamp applications and make referrals as necessary
• responsible for handling crisis intervention and coordination with the Director and Town Nurse as needed
• Identify the need and develop new programs for community outreach services
• Work independently and in conjunction with other human service agencies and specialist
• maintain confidential client records
• prepare monthly and annual reports detailing outreach activities and services

Must possess a valid MA drivers license. Must have a Bachelors Degree in social work, gerontology or human services and three to five years experience working with the elderly in a social service environment or related field.

Must be proficient in Microsoft Word and Excel. CPR/AED certification required.
