The JACQUES Initiative, the Institute of Human Virology’s HIV-focused community outreach and engagement program, is seeking a RN Care Navigator to join our dedicated Linkage to Care team.
The RN Care Navigator will work as part of a multidisciplinary team performing navigation services for people living with HIV (PLH) in our Linkage to Care program. The Nurse is responsible for linkage activities for the most medically complex patients our team engages while admitted to University of Maryland affiliated hospitals in Baltimore City and supports safe discharge as well as engagement and retention in HIV primary medical care.
The RN Care Navigator will work in collaboration and continuous partnership with the medically complex PLH and their family/caregiver(s), clinic/hospital/specialty providers and staff, as well as our full care coordination team to: promote timely access to appropriate care, reduce emergency room utilization and hospital readmissions, improve quality metrics, create and promote adherence to care plans, and increase patients’ ability for self-management and shared decision making. This role is patient focused, outcome-oriented, and based on general and specialty professional standards, and functions within the multidisciplinary practice model.
This role will provide the opportunity to contribute to expanding and refining navigation services, working on differentiated service delivery, supporting clinical services within community settings, and contributing to both quality improvement and implementation science endeavors. It is ideal for an individual who appreciates the opportunity to work in diverse environments – a typical day might include an AM huddle in the office, engaging patients in the hospital in the morning, then performing home visits in the afternoon!

Essential Functions:
Qualifications:
Education: Bachelor's degree in Nursing, Master’s degree preferred.
Experience: Three (3) years of experience nursing, clinical, or in a relevant research specialization related to the position.
Licensure/Certification:
Current Maryland license in good standing issued by the Maryland Board of Nursing to practice as a registered nurse or a multistate licensure privilege to practice as a registered nurse in Maryland or appropriate federal district licensure where research is conducted.
Other: May consider a combination of directly related experience and education.
Knowledge, Skills, and Abilities
Hiring Range: $83,000 - $98,000 (Commensurate with education and experience)
