National PLAN Alliance

The National PLAN Alliance is a network of 22 PLAN (Planned Lifetime Assistance Network)programs in 17 states created by families who have a relative with a developmental disability or mental illness. The National PLAN Alliance is available to work with any gorup of individuals who wish to establish a PLAN program in their area. Both on-sight and telephone consultation is available. The NPA is available to conduct a needs assessment and analyze the data. A guidebook for devloping a PLAN program is available for a fee. PLAN programs help families to develop an individualized plan of care that is well-thoughtout, carefully worded, legally acceptable and that will protect a loved one when family is no longer able or available to provide care. Families can arrange for PLAN to provide individualized services, contract with others on their behalf, act as trustee for the family and implement the plan of care at the present time or in the future.
