Visiting Nurse Service of NY Community Care Management Health Home

Community Care Management Partners Health Home is a community-based health home comprised of eight partners: Visiting Nurse Service of New York, Argus Community, Community Healthcare Network, iHealth, Institute for Family Health, Mount Sinai Hospital, Bright Point Health, and Urban Health Plan. The CCMP Health Home is helping individuals with Medicaid connect with primary care physicians and specialists, in addition to dealing with such social service needs as homelessness, legal issues, educational and language barriers. By connecting these patients to regular care, CCMP is helping to improve their health by providing comprehensive care coordination services to help members navigate the medical, behavioral and social service systems.
