Community Forward is a collective of programs rooted in compassion, connection, and equity. We exist to care for our unhoused neighbors—especially women, older adults, people living with chronic illness, and those navigating substance use and trauma. Together, our network offers safe shelter, emotional support, housing stability, and pathways to healing.
We meet the needs of people, rather than demanding that people meet the needs of our programs.”
The Permanent Housing Advanced Clinical Services (PHACS) program brings trauma-informed, wrap-around mental health and behavioral health support to permanent supportive housing sites across San Francisco. These services are tailored for individuals with high-acuity needs—supporting their mental wellness as they rebuild their lives in stable housing.
As the Director of Clinical Services, you’ll lead a dedicated clinical team working at the intersection of housing, mental health, and social justice. You’ll guide daily operations, nurture a thriving team culture, and ensure we continue delivering compassionate, evidence-based care.
Your leadership will directly impact the mental wellness and housing stability of our community’s most marginalized members.
