To be the healthcare provider of choice in upstate New York and to create healthy communities in which health disparities are diminished and there is access to quality care.

Mosaic Health has been working for six decades to provide greater access to vital health care services to underserved patients of all ages, ethnicities, and backgrounds. That mission has only expanded as we’ve opened health centers throughout New York State, from the Finger Lakes Region to the Mohawk Velley.

Through the years, Mosaic Health has partnered with dozens of other organizations in New York to address issues ranging from lack of access to care for people of color, lead poisoning in children, HIV treatment and prevention, and exercise for seniors. Through it all, our mission, our vision, and our values hold true.
