    We’re excited to host an Onsite Interview Day for the Bilingual Spanish Case Manager role in our Community Guardian Program! Candidates whose applications are selected will have the opportunity to interview directly with the hiring manager. If selected, you will be contacted to confirm your scheduled interview time slot.

    Schedule & Hours:

    • Shift: Monday–Friday, 9AM–5PM

    • Hours per Week: 35

    • Location & Travel: The position will involve 2 days a week in our Manhattan office and a minimum of 3 days traveling to various locations throughout the Bronx AND Manhattan for cases. In emergency situations, additional travel may be required.

    About the Community Guardian Program:JASA serves as a legal guardian for adults deemed legally incapacitated by the courts. Case Managers provide comprehensive care through assessments, service planning, advocacy, and ongoing support to ensure clients’ needs are met—ranging from medical and social services to housing, financial support, and daily living assistance.

    What You’ll Be Doing:As a CGP Case Manager, under the guidance of a Master’s level supervisor, you will:

    • Conduct initial and monthly in-home assessments for clients and families

    • Screen clients for program eligibility and provide crisis intervention when needed

    • Develop and implement comprehensive service plans

    • Arrange and oversee direct services (home health, public assistance, legal aid, medical care, housing, etc.)

    • Disburse funds through JASA programs as appropriate

    • Ensure compliance with guardianship court orders

    • Advocate for clients and collaborate with community agencies

    • Maintain accurate records, reports, and statistics within required timeframes

    Qualifications:

    • Bachelor’s degree in Social Work, Sociology, Psychology, Human Services, or equivalent

    • Bilingual in Spanish and English

    • 1+ years of case management experience preferred

    • Strong organizational, time management, and computer skills

    • Ability to travel to clients’ homes via mass transit

    • Sensitivity to cultural, religious, and individual diversity
