Pristine Hospice

Pristine Hospice provides personalized hospice care to patients and families with a terminal illness and six months or less to live. Hospice care is often provided in the comfort of the patient's home, or wherever the patient calls home. Home may be a private residence, group home, assisted or independent living, or skilled nursing facility. Pristine Hospice provides medicines, medical care, and medical equipment related to the patient's hospice diagnosis. Together, the patient, family, and hospice team of professional develop a plan of care tailored to the specific needs of the patient and family. The Pristine Hospice team consists of a nurse, physician, medical social worker, chaplain, certified nursing assistant, and a volunteer.
