Your professional Hospice Team includes physicians, registered nurses, home health care aides, medical social workers, therapists, counselors and volunteers. The team develops a unique plan of care to support you and your caregiver. A registered nurse will lead your Hospice Care Team, managing your care, making regular visits to your home, scheduling visits of other care providers and communicating with your physician. Our nurses assist in pain relief, provide instructions for taking medication, and suggest nutrition therapies, as needed. They can also change dressings, care for wounds and provide home intravenous therapy. Help negotiating the healthcare system. Our nurses will coordinate with local health care resources for medications, medical supplies and referrals for basic care: Our health care aides can assist with bathing, shampooing, dressing, and caring for skin and nails. Respite: Our trained volunteers can run errands or sit with your loved one so you can shop, visit with friends, or go to church. Financial assistance or additional community resources. Our medical social workers can provide financial counseling and assist individuals and family.
