    Position Title: Reentry Case Manager

    Reports To: Law and Reentry Program Manager

    Community: San Mateo County

    Office Location: Redwood City, CA (up to 50% hybrid role of in-person and remote)

    Compensation: $28.19/ hourly, generous benefits package

    Status: Full-time, non-exempt

    Reentry Program Case Manager - Position Summary

    The Reentry Program Case Manager offers structured support and guidance to youth while on probation or incarcerated at SMC Juvenile Hall, when needed. Case managers will be conducting 1:1s to recruit for the program. Upon release, case managers work with youth to set goals around education, building life skills and constructive uses of time, in order to prevent recidivism.

    Position Responsibilities

    Provide ongoing case management for incarcerated youth

    • Meet with youth regularly to build rapport and conduct assessments
    • Support and coach youth as they prepare for Reentry services and community integration
    • Enter data to track youth progress throughout the program

    Plan Activities for Clients

    • Identify and collaborate with representatives from community-based organizations to plan youth activities
    • Build and maintain stakeholder relationships (i.e. representatives from community-based organizations, corporations, and donors)
    • Prepare agenda for activities and projects
    • Provide clients with transportation to and from activities, probation-ordered programs, and events as needed.

    Provide ongoing case/crisis management for 10-15 Reentry Program Youth

    • Meet with each youth on caseload for 2-4 hours per month depending on the phase of the program (this includes both group and individual contacts)
    • Provide crisis management and support to clients as needed. Client issues often include the following: depression, education, employment, family, gangs, homelessness, incarceration, substance abuse, self-esteem, suicide, and violence
    • Conduct intakes with each youth on caseload (which will include a Service Learning Plan and a variety of assessments)
    • Assist clients and families with locating and receiving outside services to address identified needs
    • Provide general Programmatic and administrative support

    Engage In Community Meetings/Events

    • Identify ways to participate and engage in community events and meetings to understand the needs of the community in which the clients live
    • Attend and actively participate in regular community meetings, MDT’s, as needed

    Compensation & Benefits

    This position is a full-time non-exempt role at a rate of $28.19/hourly. Our Total Rewards Package also includes the following:
