    ORGANIZATION: PHILIPSTOWN BEHAVIORAL HEALTH HUB (“The Hub”)

    Putnam County (Philipstown / Cold Spring)

    SALARY: $55,000

    WHO ARE WE?

    Known as “The Hub”, the Philipstown Behavioral Health Hub is a non-profit organization that provides local access to behavioral health and addiction resources in Putnam County, NY (Philipstown region).

    Over the past decade, the region has lost many lives to fatal overdoses. Many more residents struggle with issues related to depression, anxiety and addiction. Local residents have difficulty finding and accessing critical treatment and prevention resources, because most services are located outside the community.

    The Hub was created to give residents a place to turn for local access to these resources. The Hub provides a single point of entry to mental health and addiction programs, services and education within the greater Philipstown area. All the services we offer are free.

    The Hub is young, just five years old, and it is preparing to expand its work in many ways, including more space for programming. These new opportunities require additional enthusiastic and committed staff members!

    How do we do this?

    1. We coordinate resources and service providers to ensure that our residents can get the help they need. We provide personalized and confidential support to participants and their families.
    2. We host events to educate the community, raise awareness, and reduce the stigma associated with issues of mental health and addiction.
    3. We plan and implement programming and support groups based on the current needs of members of the community.
    4. We partner with community-based entities, such as local school districts, businesses, law enforcement, medical providers and non-profits to create innovative programming.

    WHO ARE YOU?

    You have a passion for helping people to help themselves and/or their families through crises, and you are comfortable working with people from a variety of socio-economic backgrounds. You understand how diversity characterizes and shapes the human experience. Specifically, you are interested in mental health issues and in the challenges that individuals with addictions face.

    You want to deliver high-quality behavioral health care to the community, and you are proud to participate in the effort to incorporate and sustain best practices regarding clinical and non-clinical care within an organization.

    You enjoy work that challenges you on a daily basis and allows you to fully educate and engage both with participants and the broader community, including various organizations and institutions. You are responsible, organized, efficient, and willing to do various tasks. You are able to perform multiple assignments simultaneously, and you can work both independently and in a team setting.

    You welcome the opportunity to expand the capacity of an organization in delivering the best service to its clients through quality grant-funded programming and services, and you understand the need for evidence-based strategies to achieve positive outcomes.

    You have ability and interest in facilitating various support groups and you can coordinate a programming schedule.

    You have marketing skills and are familiar with Canva (or similar) as well as social media platforms. (Preferred.)

    You appreciate working with talented colleagues as passionately dedicated to compassionate excellence as you are. You have great communication skills (effective/active listening, verbal and written), and you have the ability to deal tactfully and diplomatically with other Hub employees, government officials, professional groups, participants and their families, and the general public.

    WHAT IS THE POSITION?

    The COMMUNITY & CRISIS CARE COORDINATOR (“CCC”) is a relatively new position at The Hub, reporting to the Executive Director and working with other Care Coordinators. The CCC provides comprehensive care coordination, individual and family support services, and connections to community and social supports through participant-centered care planning. The CCC, under the supervision of the Executive Director, provides services consistent with the policies and procedures of the organization. This individual is sensitive to the fundamental components of recovery, in a person- and family-centered approach that incorporates cultural appropriateness. This individual ensures that the organization’s mission, goals, and objectives are accomplished by fulfilling job and team responsibilities, as assigned.

    • The CCC engages the Hub’s participants and support systems/providers by managing intake and engaging in ongoing, personalized relationships. Ongoing relationships include goal-setting, progress reporting, and follow up. The CCC obtains knowledge of treatment options and referral sources. The CCC enters and updates participant info including IntakeQ notes, metrics reports, and case review sessions.
    • The CCC works with the team of other care coordinators to build and maintain a comprehensive resource list. This entails ensuring effective working relationships with providers and having knowledge of each providers’ range of services including discharge planning and after-care for participants. It may entail visiting provider sites.
    • The CCC is responsible for ensuring the timeliness of the Hub’s follow-through with all open participant cases.
    • The CCC may complete internal and external funding requests.
    • The CCC collects data, records notes, and prepares reports as needed. The CCC provides metrics for but not limited to: grant applications, newsletters, and marketing.
    • The CCC offers direct support for the work of other care coordinators, is available for consultation with care coordinators, and takes an active collaborative role in cases with complexity or urgency.
    • When a crisis occurs, the CCC is responsible for communication with affected individuals and families as well as with relevant agencies for collaboration with participants for programming or services. The CCC is available to help those most intimately involved with a crisis by developing a personalized plan of care, when desired. The CCC handles these, and all cases, with a deep understanding and respect for privacy and confidentiality.
    • The CCC communicates with the ED about current cases including crises or difficult community situations.
    • The CCC will participate in case conferences, trainings, workshops, seminars, and county meetings, and will meet with colleagues as needed.
    • The CCC is responsible for initiating, executing and sustaining organized activities and programs involving participants, community members and community-based organizations/ institutions. This includes (a) the generation of ideas for additional services and programs and/or modifications to our existing services, programs, and systems; and (b) suggesting best practices regarding consumer satisfaction and client outcomes and (c) leading/facilitating groups as needed.
    • The CCC is responsible for researching and generating material to be used in grant proposals relevant to client-participant care and/or Hub programs, activities and events.
    • The CCC creates, plans, and handles follow-up with programs and events; creates marketing materials including flyers and graphics for event and programming promotion; posts on social media and creates content for email blasts and newsletters; creates a monthly calendar of events; updates the website regularly; and distributes promotional materials to appropriate institutions.

    WHAT ARE YOUR QUALIFICATIONS?

    • You possess a Bachelor’s degree in social work or psychology or a related field.
    • You have earned one or more of the following license designations: LCSW, LICSW, LMHC, LMSW, CASC, or LMFT. (Preferred by not required.)
    • You have at least two years of relevant experience, including community mental health, addictions/substance abuse, and crisis intervention. (Previous experience within Putnam, Duchess, Orange, Rockland or Westchester Counties is preferred, but not required.)
    • You are familiar and comfortable with individuals from various cultural backgrounds.
    • You are facile with and sensitive to the fundamental components of recovery, in a person- and family-centered approach that incorporates cultural appropriateness.
    • You have a valid driver’s license.
    • You own or have regular access to a vehicle.
    • You are able to work some evening and weekend hours and have a flexible schedule at least a couple days every week.
    • Residence: Ideally, you reside in either southern New York State, northern New Jersey, or southern Connecticut.
    • “Lived experience”with mental health issues or addiction/recovery preferred but not required.
    • Languages: English. Spanish is preferred but not required.

    WHAT DO WE OFFER?
