    Position Title: Community Health Navigator (CHN)

    Employment Status: Part-Time, Non-Exempt (up to 24 hours per week)

    Position Type: Temporary, Grant-Funded

    Position Duration: Through November 30, 2026

    Start Date: Immediate

    Work Location: Los Angeles County (community-based work)

    Compensation: $26.60 per hour

    Reports To: Director of Community Action

    Supervises: None

    Job Summary:

    Pilipino Workers Center is looking for one (1) Part-Time Community Health Navigator. This is a grant-funded, temporary position through the end of November 2026 aimed at facilitating the dissemination of information and providing referrals and navigation support related to immigration and emerging public health issues.

    The Community Health Navigator (CHN) helps bridge community members to immigration, health, mental health, and social services information to enable them to access the services they need. The CHN will be trained to be knowledgeable about immigration programs, public health campaigns, and services provided by Los Angeles County and other community organizations.

    The CHN will support community members through outreach, education, referrals, and navigation assistance, helping them successfully access community resources and connect with the PWC community. The position also requires accurate documentation of outreach activities, referrals, and follow-up outcomes using grant-required reporting tools.

    Duties and Tasks:

    • Conduct outreach activities (community canvassing, presentations, tabling at local events, etc.) to disseminate information and provide education and navigation support to immigrant individuals and families through coordinated, community-based CHW/P/R services that strengthen linkages to immigration legal services and related health, mental health, and social supports
    • Organize at least one (1) legal clinic to support community members in accessing immigration legal services
    • Provide educational materials and conduct workshops on immigration programs and services in a culturally responsive and community-centered manner
    • Assist community members with registering for service appointments and successfully navigating resource program application processes through one-on-one support and community events
    • Provide navigation assistance and warm handoff support, which may include assisting with applications, making calls together, scheduling appointments, coordinating with service providers, and helping address barriers to accessing services
    • Maintain and distribute an up-to-date list of available immigration, mental health, and other health services, including immigration legal services, mental and social services, health insurance coverage, and other services
    • Work with the Community Services Director to maintain and expand the PWC immigration attorney and immigration legal services organizations referral list
    • Provide outreach, education, and promotion for emerging public health campaigns
    • Participate and contribute in the Rapid Response Team of PWC
    • Train and coordinate with the PWC Neighborhood Caring Communities to enable volunteers to understand and access County programs and services.
    • Assist community members affected by immigration concerns
    • Refer and collaborate with other PWC programs and services to achieve PWC’s larger goals of community and worker empowerment
    • Transport community members to meetings, events, and/or service appointments as needed
    • Prepare outreach materials and educational resources
    • Help set up and support community events
    • Conduct phone banking to support outreach and engagement activities
    • Follow up on referrals to support participants and document whether services were successfully provided, supporting closed-loop referrals whenever possible
    • Assist in organizing activities within the community
    • Complete administrative tasks such as recording, logging, and documenting outreach activities, referrals, participant interactions, and follow-up outcomes using grant-required reporting systems and forms
    • Track outreach activities including type of outreach, number of individuals reached, languages used, and materials distributed as required by grant reporting guidelines
    • Assist with collecting community feedback from participants to help document community impact, service satisfaction, and barriers to accessing services
    • Other duties and functions as determined by organizational needs

    This position is supported by grant funding. Duties may be adjusted as needed to ensure compliance with grant deliverables, reporting requirements, and program objectives established by Pilipino Workers Center and the funding agency.

    Required Skills and Qualifications:

    • At least six (6) months of experience working with the public or with community groups performing duties such as interviewing clients or community members concerning health or social services matters, answering questions, and providing information about immigration, health, mental health, and social services to community members
    • Outstanding communication and interpersonal skills
    • Ability to research information and explain it in practical and understandable language
    • Ability to support community members in navigating services with patience and cultural responsiveness
    • Ability to accurately document outreach activities, referrals, and follow-up information using reporting tools and data collection systems
    • Ability to maintain confidentiality and consistently document participant information using approved identifiers such as initials or aliases
    • Ability to drive and work at different locations within the county
    • Ability to converse and communicate in English and Tagalog
    • Possess basic computer skills including data entry and the ability to quickly learn and adopt new applications or reporting tools

    Preferred Qualifications:

    • Knowledge about immigration issues, including knowledge about immigration policy, current events, and a good understanding of the existing immigration system
    • Familiarity with health, mental health, and social service resources available in Los Angeles County
    • Experience with community outreach, workshops, referrals, or navigation services
    • Experience working with immigrant communities or culturally diverse populations
    • Experience with program documentation, outreach tracking, or grant reporting systems
