SUMMARY OF POSITION
The County of Monterey Health Department is currently seeking a motivated individual to fill one (1) Health Program Coordinator (HPC) position within the Public Health Bureau’s Enhanced Care Management Program (ECM). The Health Program Coordinator is a full- time, regular, staff-level professional position reporting to the Public Health Program Manager I.
The Health Program Coordinator will administer critical public health programs in the Enhanced Care Management (ECM). The Enhanced Care Management Program serves clients who are experiencing homelessness, have chronic medical conditions, have avoidable emergency department and/or hospitalization usage, have severe mental health diagnosis or challenges with substance use, and pregnant and postpartum individuals who are subject to racial and ethnic disparities. The Health Program Coordinator (HPC) will specialize in knowledge of 1-2 populations above. In this position, the incumbent will hire and train new team members, assign and review the workload of the professional team, and coordinate with partners. The Health Program Coordinator will directly supervise professional staff in the Enhanced Care Management team such as Community Service Aides II, Behavioral Health Aide and/or clerical staff. The HPC in ECM functions more as a program support role. This will include the evaluation of program effectiveness to ensure ECM’s daily operations comply with both long and short-term program goals and objectives and agency policies and procedures.
The Health Program Coordinator will development and maintain effective working relationships with service and healthcare providers and community groups; planning, coordination, maintenance, and evaluation of programs; engagement of stakeholders from the community and potential partnership organizations with regards to prevention, detection, and response to public health issues; and development of program goals and objectives with stakeholder input. The HPC uses principles of reflective practice and staff development to provide primary support and appropriate oversight to the home visitors to build and model a strengths-based client-centered culture.
